Addressing Social Risks in Cancer Survivorship: An Interdisciplinary Teams Perspective

Purpose
Understanding and strengthening how interdisciplinary teams integrate, collaborate, and coordinate social care into cancer care is essential for improving cancer-related outcomes and achieving health equity. In this interactive session, Dr. Ray Osarogiagbon will guide an esteemed multidisciplinary panel through a case study illustrating the challenges faced by a person diagnosed with cancer from treatment to follow-up care. Panelists will discuss team-based care approaches to care coordination, system-level barriers and facilitators of interprofessional collaboration, and opportunities for value-based cancer care.

This Webinar Series is intended for clinicians, researchers, community and healthcare leaders, people living with cancer, survivors, advocates, health information technology experts.
