You must have an IMS/NIH account in order to submit a request for the SEER-Medicaid data. If you do not have an account, please use the link on the Log In page to register for one. Your email address will be the primary identifier for this account and will be used for all communications about your request. Please be sure to use a valid accessible email address.

You will need to log into your account and complete the profile information to Submit a SEER-Medicaid Data Request.

The View My Requests page can be used to access all submitted requests.
